Habilitation Specialist 1 Qualified Intellectual Disability Professional (QIDP)

The Habilitation Specialist 1 (HS1) position is responsible for helping residential participants reach their optimum level of functioning by developing individual habilitation plans, setting goals and monitoring an individual's progress in reaching these goals. The HS1 will responsible for our individuals who have varying degrees of functioning levels and behavioral involvement. The HS1 will be based out of the Depot Hill IRA and provide oversight for 6 IRAs. These locations are subject to change.
Primary Duties and Responsibilities
The Habilitation Specialist 1's duties will include, but are not limited to:
o Will write Staff Action Plans that address the outcomes as reflected in the Life Plan, as well as the needs and interests of assigned individuals o Train all IRA staff in implementation of Staff Action Plans, data collection and continuing notes to substantiate Res-Hab billing o Plans will address independence, integration, individualization and productivity o Develop, revise as needed, and ensure implementation of individual-specific Safeguard Summary Plan o Provide oversight to assigned homes and bring issues/concerns to the attention of appropriate team members; o Follow up with house managers and assist house managers in training and supervising all direct care staff in all areas related to Residential Habilitation o Ensure all Residential Habilitation documents are maintained in the individuals' records, in proper order, and that records are neatly maintained per regulation and policy o Develop and oversee individuals' Personal Expenditure Plans, ensuring that individuals' desires and interests are met and money is budgeted o Provide input to House Managers and/or Recreation staff in the planning of community inclusion activities that are appropriate to each individual's interests and abilities o Other duties as assigned

Salary Range:
From $45455 to $58028 Annually

Qualifications:
Candidates must have QIDP certification/status and permanent service as a Habilitation Specialist 1 OR must be active and reachable on the current Civil Service eligible list for the Habilitation Specialist 1. Transfer
Qualifications:
Candidates must have QIDP status/certification and be eligible for transfer under Section 70.1 of the Civil Service Law. Provisional
